Direct Services for North Pima County Health Outreach:
1. Works to increase the recruitment and enrollment of eligible families in AHCCCS and other medical homes.
2. Provides outreach and education to all families with young children regarding comprehensive, preventative health care, including the need for immunizations, oral health care, nutrition, exercise, and regular well-baby checks.
3. Provides outreach and education to health care providers, including pediatricians and dentists, for the purposes of increasing the usage among physicians of standardized developmental screenings and securing dental homes for young children.
4. Supports health outreach staff in effective use of resources.
5. Collaborates with community-based agencies and community health centers in provision of services.
Leadership and Supervision
1. Supervises health outreach staff with respect to their health outreach duties.
2. Assists in the development of education materials.
3. Develops policies and procedures regarding health outreach and enrollment support.
4. Works with health outreach staff to establish referral and follow-up systems regarding enrollment in health insurance opportunities and community health centers.
5. Develops strategies for providing parent education regarding comprehensive preventative health care throughout North Pima County.
6. Establishes strategies for working with health care providers for purpose of promoting the use of standardized developmental screening tools.
7. Develops strategies for supporting families in finding dental homes and encouraging more dentists to provide services to young children.
8. Collaborates with agency’s Administrative Information Manager in development of all record-keeping systems, such as recruitment, enrollment in medical homes, parent participation in education activities, and provider outreach.
9. Monitors and analyzes monthly reports of health outreach staff.
10. Keeps administrative staff and liaisons informed of activities and progress.
11. Participates in Health and Nutrition Services Advisory Committee, and HNS staff meetings as appropriate.
Community and Professional Relationships
1. Establishes and maintains collaborative relationships with a wide variety of community-based agencies, community health centers, preschools, and childcare centers in North Pima County.
2. Participates in Early Childhood Partnership Council in North Pima County.
3. Participates in First Things First (FTF) partnership meetings as appropriate.
4. Seeks collaboration with all FTF partners in North Pima County.
Reporting and Record-Keeping
1. Establishes reporting system for health outreach staff.
2. Monitors reports from health outreach staff.
3. Establishes system for tracking parent education activities, including outreach and services delivered.
4. Completes monthly reports for agency and FTF.
5. Participates and supports FTF evaluation activities.
6. Establishes and maintains current community resource and referral file.
Training
1. Provides outreach and enrollment support for Health Outreach workers.
2. Participates in and provides training for on-line AHCCCS enrollment.
3. Presents parent education activities regarding comprehensive preventative health care, including oral health, nutrition, well-baby care, immunizations, exercise, etc.
4. Assists in preparation of parent education materials.
Responsibilities for all Health Outreach Staff:
1. Understands and models the agency's vision and mission in the agency and the community.

6. Participates in required training and continuing educational opportunities.
7. Follows all required rules and regulations.
8. Performs other duties as required by supervisory staff.
